COVID-19 Vaccine

JPS is currently offering COVID-19 vaccinations to our patients:
  • 65 years of age and older.
  • 18 years of age and older with at least one chronic medical condition that puts them at increased risk for severe illness from the virus that causes COVID-19.

  • *As defined by the state, chronic medical conditions include, but are not limited to: cancer, chronic kidney disease, COPD (chronic obstructive pulmonary disease), heart conditions such as heart failure, coronary artery disease or cardiomyopathies, solid organ transplantation, obesity and severe obesity, pregnancy, sickle cell disease, Type 2 diabetes mellitus.

    Please sign in to your MyChart account or “Sign Up Now” and “Schedule an Appointment” to choose the time and location of your vaccination.

    Virtual Routine and On-Demand Urgent Care visits are now being offered. Log into your MyChart and choose talk to a doc through your menu.


    JPS Connection is our county assistance program that helps eligible Tarrant County residents by providing quality healthcare at an affordable cost. The program does require a completed application and supporting documents to determine eligibility. Once eligible, coverage is provided and can only be used at JPS facilities.
